Student Grievance Form
The Student Grievance Form provides students with a formal procedure to make claims that their rights under the law or university policy have been violated. Students are encouraged to attempt to resolve issues at the appropriate level of dispute; however, should this approach fail or be inappropriate, students may submit a grievance through the Student Grievance Form. If you are enrolled in an online course or program, please complete and submit the NCCU Online Student Grievance Form. You will receive confirmation that your grievance was received and routed to the appropriate office within 24 hours. Please carefully review the Grievance Procedure for Undergraduate Students or the Grievance Procedure for Graduate and Professional Students as appropriate.

Student Complaint Form
The Student Complaint Form provides students with an avenue to submit a concern regarding an academic or a non-academic matter. Students are encouraged to resolve complaints at the appropriate level of dispute; however, should this approach fail or be inappropriate, students may submit written complaints through the Student Complaint Form. If you are enrolled in an online course or program, please complete and submit the NCCU Online Student Complaint Form. You will receive confirmation that your complaint was received and routed to the appropriate office within 24 hours.

Information about Student Complaints
The State Authorization Unit of the University of North Carolina System Office serves as the official state entity to receive complaints concerning post-secondary institutions that are authorized to operate in North Carolina. If students are unable to resolve a complaint through the institution’s grievance procedures, they can review the Student Complaint Policy and submit their complaint using the online complaint form.
