Cheatham-White Scholarship School Endorser Nomination Form


Instructions

Nominations must be received no later than November 1 of the student's senior year and endorsement must come from an eligible High School Guidance Counselor, High school Principal, Assistant Principal, Head of School or designee. Incomplete forms will not be considered.
  • Up to 199 seniors: 2 nominees
  • 200-399 seniors: 3 nominees
  • 400-499 seniors: 4 nominees
  • 500 or more seniors: 5 nominees

Students are unable to complete an application for the Cheatham-White Scholarship unless the School Endorser Nomination Form has been submitted.

Students must have applied for admission to North Carolina Central University and be admitted to be considered for the Cheatham-White Scholarship.

Once the nomination form has been received, applicants will receive a notification of nomination for the Cheatham-White Scholarship. Additionally, the applicant will be provided detailed instructions regarding the application process.

School Endorsement

To be completed and signed by a High School Guidance Counselor, High school Principal, Assistant Principal, Head of School or designee.

By submission of this nomination you affirm that the information is true and correct and consent to be contacted by a representative from the review committee to discuss your responses and provide additional information if necessary.

Deadline

A completed application must be received on or before 11/1/2018 11:59 PM for consideration of this scholarship. Finalists will be notified in mid-January and invited to complete scholarship interviews.
