Tips for filling out the application

1.  Method of Payment:  You must pay the application fee by Cashier’s Check or Money Order only.  These can be acquired at any Post Office or bank.  Please submit the fee with your application.

2. Name:  Be sure to type or print your official legal name.

3. Social Security Number: This is NOT required.  If you do not provide it, you will be furnished with an alternate identifying number.

4. Address:  Please indicate the address where you reside, and offer an address to which you would like correspondence mailed. Both permanent address and mailing address may be the same for many people.

5. Citizenship:  Put a mark in the appropriate category.  The following note is from the NCCU web site: “If your citizenship is other than U.S. and you have Permanent Resident status, enclose a photocopy of both sides of your I-551 (green card).”

6. Date of Birth/Sex:  Please fill in the appropriate information.

7. Are you a legal N.C. resident?  This question is for those students wishing to qualify for in-state tuition rates, which are less expensive.  If you have not lived in North Carolina for the past 12 months, mark “No.”  If you have lived in NC for one year, and wish to qualify for reduced tuition rates, please mark “Yes,” and complete the additional form.

8. Enrollment Classification:  Most applicants for the ELD program should mark “Unclassified/Non-Degree.”  This category includes people who wish to earn a certificate.

9. Campus housing/financial aid:  Please mark the appropriate boxes.  Note that most ELD students will not qualify for financial aid.

10. Race: This is an optional question.

11. First enrollment:  The ELD program begins in the summer.  Please mark “Summer Only” or “Summer and Fall.”  Please also indicate the year you plan to begin, but you do not need to indicate intended major.

12. High Schools and Colleges:  List any that you have attended, or write “none.”

13. Contact information:  Please fill out this information.

14. Campus Safety Questions:  These questions are required.  Please mark the correct answers.

15. Signature:  You MUST sign and date the form and include your application fee.  Without this information, your application cannot be processed.
